Alabama requires an up to date Blue Slip before
attending school at any grade level. If your child's blue slip has
or will expire a note was sent home with your child. This includes
High School students also. Please use the summer months to get the
needed immunizations to avoid missed days next school year.
5Th grade students are required to get the Tdap vaccine before
entering the 6Th grade. Information is being sent home with the
students.

cooperative efforts of the school staff, physicians, parents and the
community. Our goal is to protect and to promote the health and well
being of our students.

The following form must be completed and returned to the school by the parent or guardian of any student who requires that medication be taken while at school. This also includes any over-the-counter medications (example: aspirin, Tylenol).
