This is a voluntary wellness program free for public education
employees, retirees and adult dependents (18 & up for health
screening; *20 & up for osteoporosis scanning) who are covered under
the PEEHIP plan (includes BCBS GROUP #14000, BLUE CROSS BLUE SHIELD
SUPPLEMENT XAK, SOUTHLAND, AND VIVA - NO EXCEPTIONS CAN BE MADE)
You must bring appropriate insurance card.
Screening Includes:
• Blood Pressure/Pulse check
• Diabetes check
• Osteoporosis screening*
• Cholesterol Check from Fingerstick (includes Total, HDL, LDL &
Ratio)
• Personalized Health information
• A Take Home Colon Cancer Screening Kit for high risk participants
Mission:
The mission of Double Springs Elementary School is to create a child-centered environment conducive to the development of lifelong learners through a partnership involving parents, community, and school. We, the faculty and staff, accept the responsibility for teaching all students, and will use a variety of strategies, methods, and techniques to prepare them for the unique challenges of a changing world.
