Important Information:
ANY student requiring a prescription medication to be given at school WILL need
a Medication Prescriber/Parent Authorization Form signed by their
physician/doctor before the school nurse or designated person can accept the
prescription medication.
ANY student requiring an Inhaler to be kept on them MUST have a Student Self-Administration Form of Asthma Medication and a School Medication Prescriber/Parent Authorization Form on file with the school nurse.
These forms DO NOT carry over from year to year. Each year a new form(s) must be completed and turned into the school nurse.
