Forms for Parents

• Anyone wishing to donate medical supplies such as Band-Aids, coban, 4 x 4's, etc. would be appreciated and welcomed.
• It is important to read the Medication Guidelines before completing the following form.
• The following form must be completed and returned to the school by the parent or guardian of any student who requires that medication be taken while at school. This also includes any over-the-counter medications (example: aspirin, Tylenol).
