IMMUNIZATION REQUIREMENT: Beginning with the upcoming school year (August 2010), all students 11 years of age and older, entering 6th grade, will be required to present a Certificate of Immunization (Blue Card) verifying that their child has been immunized, after the 10th birthday, with TDaP (tetanus-diphtheria-acelluar pertussis) vaccine. Please schedule an appointment with our family physician or county health dept prior to August 9, 2010. A copy of the blue card should be returned to your child's school by August 9, 2010.

IMPORTANT: Unused Medications at School
Parents of students with medication at school will be receiving a note
from the local school nurse regarding the date for unused medications to
be picked up from the school nurse by the parent prior to the end of
this school year. (Parent/Guardian must pick up unused medication.) No
medications are carried over to the next school year. Any unused
medication not picked up by the date provided by the school nurse will
be destroyed according to school procedure.
REGARDING MEDICATIONS FOR THE NEXT SCHOOL YEAR (2010-2011):
Please remember that the school nurse must have
NEW Parent/Physician Authorization for each medication the student
is to receive at school for the new school year BEFORE the medication
can be received by the school nurse. Please review the
Medication Guidelines.
